Damage Prevention Survey by OPS

As part of a directive from Congress, the Office of Pipeline Safety, (OPS), is taking an in-depth look at certain state damage prevention programs to see what is effective in making them work. To help them in their effort, OPS would like as many Kansas stakeholders as possible to fill out this form (Microsoft Word format).

Please fill out the first page and especially the very general questions at the beginning. The remaining categories of questions can be answered based on your areas of interest in the Damage Prevention industry.
